When I get to know that my original LIC bond has gone missing, I was interpreting the impending hassle; tough my wife usually terms me as pessimistic but this time I was correct. I was not aware how to go for this, called up my agent who very sincerely told me the procedure is very easy ….. first file a General Diary in a police station of the area then call me up for further procedure compliance. And then he never turned up after several request.

1. An agreement for surety on Rs. 50 (Fifty) non-judicial stamp paper and notary
2. Query Form filled up and endorsed by Gazetted under office seal
3. Attestation of Signature by Notary
4. Form No. 3510- declaration regarding assignment
5. General Diary copy in original.
Ensure all the forms are properly filled up and endorsed where ever required. A Rs. 97/- as a charge by LIC have to deposit at the Local Branch along with all documents where the policy is registered.
